How do we work?
Basically, after applying and having a successful interview (we like to call it a "meet & greet") you straight away have access to our secret club app that you will download. About the role:
Reporting to the Head of Community Care and Home Care Services, the Clinical Lead is a senior clinical position responsible for:
- Providing leadership and clinical oversight to a multidisciplinary team of Care Managers (including RNs, ENs, and non-clinicians)
- Driving the delivery of safe, effective, and person-centred care across all community programs
- Supporting complex client care planning, reassessment, and clinical escalation processes
- Monitoring and responding to clinical risks, incidents, and compliance requirements
- Ensuring adherence to the Aged Care Quality Standards, the Aged Care Act, and the Support at Home reforms (commencing November 2025)
- Supporting onboarding, mentoring, and professional development of frontline care staff
- Acting as a key clinical interface with external providers, allied health, and regulatory bodies
What we need from you first:
- AHPRA registration, post graduate preferred
- Current Flu vaccination evidence
- COVID-19 Australian Government vaccination certificate
- Current National Police Check (not older than 18 months)
- NDIS Clearance
- Current driver's license/reliable source of transport
- Wonderful people skills- can build rapport easily with facilities and their residents while representing CBH
