Job Title: Case Manager

Award: SCHADS Level 5.1 Reports To: Team Leader

Community Accommodation and Support Agency Inc (CASA) is a registered housing provider. The organisation is in Mackay North Queensland and is locally recognised as a leader in providing tailored housing and support services to disadvantaged clients to achieve independence, stability and self-reliance.

Purpose of Position:

To ensure the delivery of high-quality service to clients through the implementation of quality case management systems, practice standards and service delivery to work with families and individuals who are homeless or at risk of homelessness that will result in maximising their capacity to be independent, self-reliant and connected to appropriate social and community supports.

Key Responsibilities:

  • Assess the needs of clients and provide appropriate support to individuals and families whose tenancies are at risk.

  • Support change to achieve long term sustainable outcomes for clients.

  • Participate in case management strategies with clients and other service providers.

  • Maximising continued improvement for delivery of services by the organisation to the community.

  • Ensure consistency in delivery of individual tasks.

  • Interpret the organisations relevant policies and procedures relating to engagement with clients and carrying out daily responsibilities related to your particular position.

  • Providing analytical client contact reports.

Mandatory Requirements:

  • Minimum Cert III in community services or other relevant qualification

  • Current Class C Drivers Licence

  • Blue Card Qld (Clearance to work with children), NDIS Workers Screening Clearance Card

Desired Requirements:

  • Minimum 3 years' experience within the community services sector

  • Previous Housing/RTA knowledge

Essential Selection Criteria:

  • Demonstrated strong analytical and case management skills relating to assessment, intake, case planning, case reviews, co-service delivery and critical incident responses.

  • Demonstrated ability to ensure self-compliance with organisational and funded bodies regulations, policies and procedures.

  • Excellent verbal and written communication skills including the capacity to use Microsoft Office Software (Word, Excel and Outlook) and/or ability to learn and/or navigate the organisation's data bases and funding bodies requirements.
