Case Partner plays a key role in the new Support at Home Program, supporting older Australians to access and navigate the services they need for their individual care. This position focuses on delivering person-centred care planning, coordinating services, and building strong relationships with participants and their support networks. The aim is to enhance the quality of life for individuals in the community. The Case Partner ensures that participants receive the most appropriate, timely, and effective support aligned with their personal goals.
The Case Partner is responsible for managing a caseload of participants within the Support at Home Program. This includes coordinating and overseeing the delivery of their care services. The role requires working closely with participants, their families, service providers, and other health professionals to ensure care plans are tailored to each person's individual needs and preferences.
QualificationsQualifications:
- Certificate IV in Aged Care or a relevant qualification in Nursing, Allied Health, Social Work, or a related field is required.
- A degree or equivalent experience in aged care is highly preferred.
Experience:
- Demonstrated experience in case management or service coordination, especially within aged care or community care sectors.
- Previous experience working with the aged care system is an advantage.
Other Requirements:
- Current Australian Driver's License.
- National Police Check and Working with Children Check (or willingness to obtain).
- First Aid and CPR certification.
- A commitment to continuous professional development and training.
- Excellent Communication & Interpersonal Skills: Able to build and maintain positive relationships with families, participants, and service providers through clear, respectful, and empathetic communication
- Problem Solving & Decision Making: Strong analytical skills with the ability to assess situations, identify solutions, and make sound decisions in a timely manner.
- Organisational Skills: Highly organised with the ability to manage multiple tasks, prioritise workload effectively, and maintain attention to detail in a fast-paced environment.
