About the Role:

As a Restorative Care Partner, you'll be a trusted advocate, guide, and connector in each participant's wellbeing journey. You'll play a key role in driving proactive, person-centred support by:

  • Engaging with participants, families, and community stakeholders
  • Conducting in-depth assessments and coordinating personalised restorative care plans
  • Providing ongoing advice, guidance, and care navigation
  • Ensuring quality care delivery, risk management, and continuous improvement

In this role, you will:

  • Conduct home visits to understand each person's unique needs and aspirations
  • Develop tailored care plans that empower individuals to take control of their health and lifestyle
  • Lead and collaborate with a multidisciplinary team to deliver seamless, integrated services
  • Build genuine, trust-based relationships with clients and their families
  • Champion quality, innovation, and excellence in care delivery

About You:

You're someone who wants to make a real, lasting difference. You bring compassion, leadership, and a strong understanding of restorative care.

You have:

  • Tertiary qualifications in Allied Health, Nursing, or a related field is essential
  • Experience in restorative care, allied health, or community services
  • Experience in care coordination, restorative or palliative care (highly desirable)
  • Knowledge of Aged Care Quality Standards, Support at Home program, and rights-based care
  • A proactive, person-first approach and the ability to connect with empathy
  • Strong communication and leadership skills within team-based environments
  • Confidence to work independently while contributing to a supportive, collaborative team
  • A commitment to learning, growth, and best-practice care
  • In-depth knowledge of reablement, functional rehabilitation, and person-centred care
  • Strong IT skills, including proficiency in Microsoft Word, Excel, and client management systems
  • Budget Reporting Experience, or willingness to learn

You're also proficient in:

  • Risk identification and management
  • Quality assurance and compliance
  • Assessment tools and outcome-based care planning
