You'll work directly with NDIS participants, helping them navigate daily life, achieve their goals, and build meaningful connections within their community. Every day brings new challenges and rewards as you support individuals on their journey toward independence and wellbeing.

Your Key Responsibilities

Direct Client Support

  • Engage clients in meaningful community activities and social connections
  • Provide assistance with domestic tasks including light cleaning, cooking, and meal preparation
  • Offer safe, reliable transport to appointments and activities
  • Facilitate uplifting, trauma-informed conversations that promote healing and growth

Professional Documentation & Collaboration

  • Maintain accurate, detailed case notes and progress documentation
  • Prepare incident reports when required
  • Collect and implement data for allied health professionals
  • Follow individualised Case Management and Behavioural Support Plans
  • Collaborate effectively within our multidisciplinary team including clinical staff, house managers, families, and guardians

Goal Achievement & Advocacy

  • Support participants in working toward their NDIS goals
  • Advocate for client rights, preferences, and autonomy
  • Contribute to positive outcomes through consistent, person-centred care
