We're hiring compassionate and motivated Social Workers to join our team for a particular Participant.

A Social Worker provides assessed, strengths-based, client-centered social work services to individuals with complex needs, with a particular focus on drug and alcohol (substance use) resourcing and supports. The role aims to facilitate autonomy, advocate for access to appropriate services, coordinate multidisciplinary care, and support harm reduction, recovery goals, and social inclusion.

Individualised Support

Conduct comprehensive biopsychosocial assessments to identify substance use needs, risk factors, strengths, and goals, with a focus on client autonomy and recovery pathways.

Develop, implement, monitor, and review person-centered support plans that address drug and alcohol concerns, mental health, physical health, housing, employment, and social connectedness.

Coordinate access to substance use treatments, harm reduction services, counselling, detox/rehabilitation referrals, pharmacotherapy when appropriate, and other community supports.

Facilitate active participation in daily routines and community activities while supporting safety, dignity, and informed decision-making.

Support clients to engage with evidence-based practices, motivational interviewing, and recovery-oriented approaches.

Provide advocacy to access funded supports under NDIS or other relevant funding streams, ensuring plans reflect current needs and goals.

Support family members and carers with education, resources, and involvement in planning where appropriate.

Collaboration and Multidisciplinary Work

Collaborate with healthcare professionals, addiction specialists, mental health practitioners, social workers, housing services, and allied health professionals to co-design integrated care plans.

Liaise with NDIS and external partners to align supports with funding requirements and service delivery expectations.

Build and maintain relationships with community organisations, support groups, and local resources to expand referral pathways.

Documentation and Administration

Maintain accurate, timely, and confidential case notes, assessments, care plans, risk assessments, and progress reports.

Ensure compliance with Equisent/Equal Living policies, ethical standards, and relevant legislation (e.g., NDIS Practice Standards, Privacy laws, and the Disability Act where applicable).

Prepare reports for funding bodies, team reviews, and client reviews as required.

Manage caseload in a manner consistent with organisational policies and best practice standards.

Safety, Risk and Compliance

Conduct risk assessments related to substance use, mental health, and guardianship or safeguarding concerns.

Implement and monitor safety plans, harm reduction strategies, and emergency procedures.

Ensure compliance with worker screening, including NDIS worker checks where applicable and whistleblowing or reporting obligations.

Professional Development and Quality

Participate in ongoing professional development, supervision, and performance reviews.

Contribute to service improvement, quality assurance, and best practice in substance use support within the organisation.

Key Attributes

Skills & Experience - Essential

· A recognised social work qualification (e.g., Bachelor of Social Work, Master of Social Work) and eligibility for professional membership where applicable.

· Experience or demonstrated capacity to work with people who have substance use issues, including harm reduction and recovery-oriented approaches.

· Understanding of the NDIS framework and relevant legislation (e.g., Disability Act 2006, privacy laws, safeguarding).

· Strong communication, interpersonal, and written skills; ability to work effectively with diverse populations.

· Ability to work with limited supervision while exercising professional accountability.

· Ability to work both independently and as part of a multidisciplinary team.

· Computer literacy and experience with case management systems and documentation.

Employee Responsibilities

· Complete a NDIS Worker Screening Check

· Maintain current First Aid/CPR where applicable.

· Understand emergency procedures and contribute to a safe work environment.

· Maintain professional boundaries, ethical practice, and safeguarding standards.

· Participate in annual performance development, training, and supervision.

· Minimise wastage and promote sustainable practices.

· Comply with Equal Living/EDS policies and procedures relevant to the role.

Qualifications and Checks

· Bachelor or Master level qualification in Social Work (or equivalent) with eligibility for relevant professional registration/credentialing.

· Current NDIS Worker Check (or equivalent) and any state-based clearances.

· Current First Aid & CPR certification.

· Knowledge of or willingness to complete relevant modules (e.g., NDIS Practice Standards, Quality, Safety, and You).

Desirable Certifications

· Certificate in Addiction Counselling or equivalent (or active study in relevant area)

· Mental Health First Aid or equivalent

· Experience with caseload management software

· Working with Children's Check (where applicable)

· Driver's licence (if required for community-based roles)
