Australian Community Aged Care (ACAC) is growing its Support at Home services and is looking for a dedicated Care Partner to join our team. This role is ideal for someone who is passionate about client-focused care and thrives in a collaborative, purpose-driven environment.

We offer a wide range of services including in-home support, transport, social engagement, and wellness programs. Our mission is to foster inclusive communities and reduce isolation through compassionate care and connection.
Role Overview
As a Care Partner, you'll be the first point of contact for clients, working closely with them and their families to ensure their care aligns with their personal goals, preferences, and changing needs. You'll coordinate services, advocate for client wellbeing, and contribute to a safe and supportive care environment.
Key Responsibilities
- Develop and manage personalised care plans and budgets in consultation with clients and Registered Nurses.
- Conduct home safety and risk assessments.
- Coordinate services with support workers, healthcare professionals, and external providers.
- Build strong, respectful relationships with clients and their families.
- Monitor care outcomes and adjust services as needed.
- Support clients in navigating aged care systems and accessing appropriate services.
- Collaborate with clinically qualified staff for complex care needs.
- Maintain accurate records and ensure compliance with Aged Care Quality Standards.

Who We're Looking For
- Certificate IV or Diploma in Community Services, Aged Care, Disability, or Case Management.
- Enrolled Nurses (ENs) are also encouraged to apply.
- Experience in aged care or community service coordination.
- Understanding of aged care standards and person-centred care principles.
- Strong communication, advocacy, and digital literacy skills.
