Just Better Care Mid North Coast is seeking a dedicated Care Partner to join our friendly and supportive aged care Support at Home team in Port Macquarie. This is your opportunity to make a meaningful impact in your local community while working with a team that values respect, collaboration and person-centred care.

The opportunity

As a Care Partner with Just Better Care Mid North Coast, you'll work closely with participants and their families, guiding them through their aged care journey with empathy and expertise. You'll help identify needs, coordinate services, and ensure each person receives personalised support aligned with their goals and available funding.

In this role, you will:

  • Assist participants with planning, assessments, and service coordination to promote independent living.
  • Conduct initial assessments and collaborate with participants to develop tailored care plans and budgets.
  • Build trusting relationships with participants and their families, ensuring services reflect individual preferences and circumstances.
  • Act as a advocate, adjusting support as needs evolve to ensure continuity and quality of care.

What you bring to the role

  • Experience in case management in aged care or community services (formal qualifications preferred but not essential)
  • Understanding of aged care services including transition from home care package to support at home
  • Exceptional customer service skills with a caring nature, strong communication focus and ability to develop trust quickly
  • Confidence in using digital systems and tools
  • Current driver's licence
