Position Title: Senior Occupational Therapist - Community Transitions Team (CTT), Justice Health Olympic Park – PFT

About the Role
The Community Transitions Team (CTT) supports people who experience severe mental illness to access the necessary care and support required to ensure a successful transition from custody to the community. The Community Transitions Occupational Therapist is part of a multidisciplinary team that works in partnership with Custodial Mental Health, primary health networks, Community Mental Health Services and NDIS providers to deliver wrap around care for people with complex needs. By understanding patients' clinical, psychosocial and behavioural vulnerabilities, and social and emotional wellbeing needs, the Community Transitions Clinician will utilise general case management and discipline specific skills to link patients to relevant services in the community and support their initial reintegration to community care.
As the Community Transitions Occupational Therapist you will carry an active case load of up to ten patients across custody and the community. You will work with referring custodial health and receiving community mental health treating teams to assess and link people leaving custody to clinical, cultural and psychosocial supports with a focused time limited intervention for up 16 weeks. This exciting role is also focused on reducing the over representation of First Nations people in custody and works alongside a number of identified aboriginal positions who provide cultural and lived experience expertise within CTT.
The Community Transitions Team is one of the three teams that comprise the Justice Health Community Forensic Mental Health Services (CFMHS). The team is community based and provides assertive prison in-reach services to support peoples' transitions back into the community. Its aim is to develop mental health care plans, establish necessary therapeutic supports for patients, and provide follow-up community care to patients leaving custody. The team is recovery-oriented and promotes positive health outcomes and community participation.
Flexible and hybrid work agreements are also possible in line with service requirements and the NSW Health Flexible Work Policy.
- The CTT is a statewide service, supporting people from several correctional centres across Sydney metropolitan and regional NSW, and liaising with all NSW Health Local Health Districts. There is a requirement to travel to various locations across NSW on occasion.

The Justice Health Olympic Park (JHOP) office is located at 10 Herb Elliot Avenue, Sydney Olympic Park and Justice Health staff are able to access discounted parking at $5 a day. Public transport is readily available including buses, trains, and ferries. Easy road access is provided via the M4 and Parramatta Road via Homebush Bay Drive. 35 km of bicycle paths surround the site and both bicycle storage and shower amenities are available inside.
