About the role
The Integrated Team Care (ITC) program supports Aboriginal and Torres Strait Islander people who live with complex chronic health conditions to access the health caree they need. This program is funded undert the Indigenous Australians HEalth Care Programme.
The program aims to:
- Better treat and manage chronic conditions amoung Aboriginal and Torres Strait Islander people,
- Improve the health of Aboriginal and Torres Strait Islander people with chronic conditions,
- Improve access to culturally appropriate mainstream primary health care services,
- Foster collaboration and support between mainstream and Aboriginal and torres Strait Islander health sectors
About you
To be successful in this role you will possess:
- Ability to assist and support Aboriginal and Torres Strait Islander people to understand their chronic halth conditions and how to manage them
- Excellent interpersonal and communication skills (written and verbal)
- Ability to manage administrative components of the role, not limited to data entry and developing perfromace reports
- Comprehensive understanding of culturally appropriate health care, or willingness to learn
- Ability to prioritise workload and demonstrated organisational skills
- Current WWCC, valid Victorian Driver's License, and clear police check
- Relevant qualifications in Allied Health, Aboriginal Health Worker, or similar
- Experience working in health sector and/or community services
- Ability to work with various stakeholders to advocate for/on behalf of patients.
Vaccination against infectious disease is a mandatory requirement of this role. An offer of employment is conditional on you providing evidence that you are currently vaccinated against COVID-19 with booster & Influenza, prior to commencing employment.
