The Patient Care - Registered Nurse, is responsible for comprehensive patient assessment, accurate documentation,
liaising with prescribers, setting treatment goals and providing patient education within a structured telehealth
consultation.
Patient Assessment and Eligibility:
- Conduct comprehensive patient assessments via telehealth.
- Perform structured pre-assessments with the Patient Care Team.
- Assess medical history, medication regimes, and conduct risk screening.
- Determine patient eligibility for medicinal cannabis treatment.
Patient-Centred Care and Chronic Disease Management:
- Provide non-judgmental and patient-centred care.
- Focus on delivering an excellent patient experience during telehealth consultations.
- Discuss medicinal cannabis within the context of chronic disease management.
- Collaborate with patients to set treatment goals.
- Act as a liaison between patients and prescribers.
- Respond to patient inquiries and escalate issues as required.
- Provide follow-up support to patients implementing their treatment plans.
- Maintain accurate, timely, and comprehensive medical records.
- Offer clinical advice as needed.
Administrative Tasks and Team Collaboration:
- Complete general clinic administrative tasks as required.
- Embody teamwork, shared ideas, and constructive communication within the team.
Objectives and Key Results (OKRs) and Key Performance Indicators (KPIs):
- Deliver on individual and set departmental OKR and KPI's as determined from time to time.
- Registered Nurse with current AHPRA registration - nil practising restrictions.
- Minimum 3 years post-graduate clinical nursing experience.
- Experience in undertaking patient assessments and care planning.
- Mental health nursing and experience with medicinal cannabis treatment is not essential but will be highly regarded.
- Previous experience in a telehealth or virtual care setting is highly desirable.
- Strong IT skills are essential in this role.
- Highly developed written and verbal communication skills
- Strong time management skills with the ability to prioritise in a dynamic environment.
