About Ravenhall Correctional Centre
Ravenhall is Victoria's largest, medium security prison for adult sentenced and unsentenced males. Its contemporary campus-style design provides a safe and humane environment that enables and nurtures GEOs Continuum of Care operating philosophy.
Ravenhall also has a dedicated Forensic Mental Health Unit and a specialised Closed Supervision Unit for prisoners with significant challenging behaviours. In recognition of the importance of Continuum of Care, Ravenhall extends its presence into the community by providing ex people in our care with access to post-release support at GEO's Community Reintegration Centre.
With a key focus on offenders who are Aboriginal and those with complex and mental health needs, they have significant opportunities to engage in work, education, programs and services, through an evidence based approach to reduce re-offending.
Through investment in opportunities for people in our care to engage in work, education and programs, prisoners are not only held safely and securely but given every chance to turn their lives around. Issues behind much of the crime in our communities – such as violent attitudes, anti-social behaviours and the abuse of drugs and alcohol – are addressed through innovative programs and interventions. At the same time helping the people in our care with day-to-day challenges such as gaining and holding down employment and finding a suitable place to live also help prevent a return to crime.
Ravenhall's unprecedented emphasis on offender rehabilitation and reintegration is evidenced through collaborative and integrated relationships with key Alliance service partners including Forensicare, GEO HealthCare Plus, The Y, Honeywell and Bendigo Kangan Institute of Tafe. Together, Ravenhall provides an integrated service system that collaboratively brings about lasting positive change in the lives of the men in our care pre- and post-release. Through these partnerships the objective of Safer Victorian Communities is achieved.
About The Bridge Centre
The Bridge Centre is a support facility that offers men the opportunity to continue their rehabilitation journey outside of prison with the support of a team of specialist post-release case-managers and community partners. It's the first facility of its kind in Australia.
The Bridge Centre is a Community Reintegration Centre that supports clients transitioning into the community. A team of specialist Post-Release Case Workers provide ongoing case-management support across the breadth of individual reintegration needs.
Clients who were engaging with a clinician and/or community agency whilst at Ravenhall Correctional Centre, also receive the opportunity to continue their engagement post-release.
About the Opportunity
The Nexus Case Manager provides intensive pre-release and post- release case-management support. During the pre-release phase Case Managers work proactively with the men to understand their reintegration needs, develop supportive case-management relationships, and coordinate development of an Individual Reintegration and Support Plan.
Post-release the Case Manager provides up to three months of assertive and intensive outreach support, maintaining regular contact to ensure effective community engagement and attendance at support services in line with reintegration and support plan.
The Nexus Case Manager is responsible for developing and maintaining a network of state-wide community service providers and referral sources to assist and support clients to successfully reintegrate.
This position is located at multiple locations including, Ravenhall Correctional Centre at 97 Riding Boundary Rd, Ravenhall, VIC 3023 and GEO'S Community Reintegration Centre. Level 1/548 Bridge Road, Richmond VIC 3121.The position will also provide outreach assistance in the community including attendance at community correctional services and community based organisations as required. Interstate travel may be required.
Key Accountabilities
- Develop professional and collaborative case-management relationships with prisoners and their families (or other support network) in order to facilitate positive community reintegration outcomes.
- Work with client's pre-release to collaboratively develop their Individual Reintegration and Support Plan identifying needs and support that will promote successful rehabilitation and reintegration outcomes.
- Complete assessments, plans and referrals within expected timeframes.
- Coordinate and support clients to implement their Individual Reintegration and Support Plans post-release including assistance with housing, education/employment, drug and alcohol and mental health treatment needs, as well as family and community connectedness.
- Support clients to effectively re-engage with their families and community through outreach activities that build capacity and self-efficacy
- Provide client transport including day of release and to post-release appointments as required.
- Support the Nexus Team Leader to review and monitor the program waitlist and allocations.
- Engage collaboratively with GEO Rehabilitation and Reintegration Services and Alliance Partner staff to maximise engagement of clients in relevant programs and services.
- Develop and maintain positive relationships with stakeholders, including community providers in order to maximise reintegration outcomes for clients.
- Provide outreach support as well as crisis support, and referrals where required.
- Support management in contributing to the strategic direction and evolution of the program through demand and opportunity identification and response.
- Ensure that exit planning supports the client's ongoing reintegration including completion an exit plan and referrals and handover to other Bridge Centre staff or community agencies prior to the completion of the post-release support period.
- Ensure efficient and accurate management of data including inputting into systems, files and reports as required.
- Ensure compliance with all Company Policies and Procedures and the Governance, Risk and Compliance Manual.
Key Selection Criteria
Required Skills and Experience:
- Tertiary qualification in Social Work, Psychology, Welfare, Human Services or other relevant field or equivalent professional experience.
- Demonstrated knowledge of the Victorian Criminal Justice System including the role of the Courts, prisons, parole and community corrections.
- Demonstrated knowledge of research and evidence related to positive resettlement and reintegration outcomes for people released from prison.
- Demonstrated experience in delivery of services to individuals from diverse backgrounds including Aboriginal and Torres Strait Islanders and clients with mental health issues and cognitive impairment (Acquired Brain Injury and Intellectual Disability).
- Previous experience working in case management including the provision of outreach services.
- Ability to coordinate access to programs and services relevant to the reintegration needs of men who have been released from prison.
- Ability to develop and maintain collaboratively stakeholder relationships and networks to facilitate positive outcomes for clients.
- Well-developed interpersonal and communication skills and attitudinal characteristics conducive to building positive, non-judgemental professional relationships with people with a history of incarceration.
- Demonstrated ability to quickly learn and navigate various information management systems.
Other Requirements:
- Ability to work within a correctional setting, which will involve direct contact with prisoners.
- Valid and current full Australian driver's licence.
- Satisfactory completion of a National Criminal History Check.
- Ability to obtain and maintain a security clearance.
- Ability to work with individuals whom have been in custody.
