Clinical Care Manager | Permanent | Rural and Remote Australia

Looking for an on‑site Care Manager role with autonomy, strong support and real community impact? Lead clinical care at a single regional or remote aged‑care facility, help deliver ageing‑in‑place care and drive continuous improvement.

Essential requirements

  • Registered Nurse — current AHPRA registration
  • Experience preparing and conducting clinical assessments, care plans, AN‑AAC and accreditation documentation
  • Experience working with older people, preferably in aged care
  • Case management including case conferencing and family liaison
  • Identifying and managing clinical risk; solid understanding of the new Aged Care Standards
  • Excellent team management, administration, computer and software skills
  • Flexible, reliable and a "can‑do" attitude

Desirable

  • Minimum 3 years aged care experience in Australia

Key responsibilities

  • Provide on‑site care management and ensure compliance with Aged Care Standards
  • Lead clinical assessments, individualised care plans, medication safety and incident management
  • Coordinate case conferences and liaise with families, GPs and allied health
  • Maintain accurate clinical records and support continuous improvement
