Kalwun Development Corporation Ltd is an Aboriginal & Torres Strait Islander community controlled organisation currently seeking to fill the following vacancy within our Health and Wellbeing Services division. Kalwun Health and Wellbeing Service delivers a primary health care service to the people of the Gold Coast & surrounding communities.

About the Role

We are looking for a motivated Health and Wellbeing Worker who is passionate about improving the health and wellbeing of Aboriginal and Torres Strait Islander people.  The successful candidate will provide a high-quality service along with contributing to the development of models of holisitic health incorporating continuity of care.  As part of Kalwun's ongoing growth and development, you will be joining a community-focused team at the Coomera clinic.

Position Duties

  • Connecting and yarning with clients and families to the service to collect, collate and update their stories
  • Provide health information with education to clients and families which promotes autonomy and choice
  • Work alongside clients and families in their goal setting and prioritisation of health and social needs ultimately implementing deadly plans
  • Responding to clients and families at times of heightened need to prevent and/or avert episodes of crisis
  • Make a positive contribution to illness prevention and promotion of health through chronic disease management, health screening, health assessments, recalls and patient education.

Essential Selection Criteria

  1. Relevant Certificate III Qualification (Aboriginal and Torres Strait Islander primary health care, Community Services, Allied Health Assistance or equivalent)
  2. Demonstrate an understanding of issues affecting Aboriginal and Torres Strait Islander people and their health
  3. Ability to communicate sensitively and to effectively engage with community built on mutual respect and regard
  4. Previous experience using patient information systems, (e.g.:  MMEX)
  5. Demonstrated ability to meet targets and performance outcomes
  6. Ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people
  7. Demonstrated computer literacy including proficient keyboard and data entry skills
  8. Ability to work effectively as part of a multi-disciplinary team
  9. Current Qld C class drivers licence.

Additional Information

  • This position requires the successful applicant to satisfy a national criminal history check and hold a current drivers' license.
  • A Blue Card for working with children is a mandatory requirement. The 'No Card No Start' law applies to this position - a Blue Card must be obtained prior to commencement.
