What you'll be doing
The Youth Health Out Of Home Care (OOHC) Nurse facilitates and implements health assessments and health management plans for young people entering Out of Home Care (OOHC) within Western Sydney Local Health District (WSLHD), and those young people who are about to reach the end of their statutory care. The position will work within a multidisciplinary and multi-agency approach with key stakeholders both internal to Youth Health, Priority Populations and within WSLHD, and externally with key government and nongovernment partners including Department of Family & Community Services – Community Services, Department of Education & Communities, Western Sydney Primary Health Network (WentWest), General Practitioners, private health care providers, non-Government organisations, as well as young people in out of home care and their carers. The position has clinical, liaison, implementation and monitoring functions.

Essential Criteria:
- Registration: Current APHRA registration – Nursing/Midwifery.
Selection Criteria:
- Current APHRA Registration – registered Nurse/Midwife, with relevant post- registration qualifications and at least 3 years' post registration experience.
- Demonstrated understanding of and experience working with young people including young people from marginalised and at risk groups.
- Demonstrated experience working collaboratively with other service providers including General Practitioners, Community Services, Non-Government Organisations and the ability to work within a multidisciplinary team.
- Demonstrated currency of practice and competence in assessing children and their families in relation to their health and social needs including an understanding of and commitment to Families NSW principles and the NSW Youth health framework.
- Demonstrated experience and knowledge working collaboratively as a member of a multidisciplinary team and with other service providers including Youth health, Allied health staff, General Practitioners, Child and Family Health Tertiary referral services.
- Demonstrated thorough understanding and ability to implement the principles of primary health care and work in partnership with families.
- Demonstrated high level communication, liaison, negotiation, time management, computer and presentation skills.
- Hold a current drivers licence valid in Australian with willingness to use for work purposes.
