Youth Health Liaison Nurse Specialist

We are seeking a skilled nurse to support young people in Out of Home Care with tailored healthcare plans.
Key Responsibilities:- Facilitate and implement health assessments and management plans for young people entering OOHC within Western Sydney Local Health District, and those nearing the end of statutory care.
- Collaborate with a multidisciplinary team and key stakeholders internal and external, including Government and non-government partners.
- Provide clinical, liaison, implementation and monitoring support within Youth Health and Priority Populations domains.
- Current APHRA registration – registered Nurse/Midwife, with relevant post-registration qualifications and at least 3 years' post-registration experience.
- Demonstrated understanding of and experience working with young people including those from marginalized and at-risk groups.
- Experience working collaboratively with other service providers including General Practitioners, Community Services, NGOs, and the ability to work within a multidisciplinary team.
- Currency of practice and competence in assessing children and their families in relation to health and social needs, including Families NSW principles and the NSW Youth health framework.
