Community Care – Team Leader/Coordinator

Hours: Monday–Friday, 9am–5pm
On-Call: Alternate weeks (flexible arrangements considered)
About the Role
We're looking for a dynamic and compassionate Team Leader/Coordinator to join our purpose-driven team at Nurse Next Door Macarthur.
This unique role offers a 50/50 blend of hands-on support and coordination. You'll ensure seamless service delivery — stepping in to cover client visits when needed and coordinating schedules to make lives better every day.
Key Responsibilities
- Coordinate and schedule caregivers to meet client needs
- Manage administrative and documentation tasks
- Build strong, trusting relationships with clients, families, and the care team

Who We're Looking For
You're a motivated, hands-on care professional or nurse ready to step into a leadership role — without losing touch with the people you support.
You'll bring:
- Certificate III in Individual Support, Aged Care or Disability, or Enrolled Nurse (AHPRA-registered)
- 2+ years of experience in community, aged care, disability, or acute settings
- Natural empathy and exceptional communication skills
- Flexibility to assist with both clinical and in-home care needs
- Confidence using mobile/desktop apps and Google Workspace
- Current driver's licence and reliable vehicle
- Willingness to participate in on-call rotation (every second week)

Required Certifications & Checks
• Right to work in Australia
• AHPRA registration (if EN)
• NDIS Worker Screening Check
• Working With Children Check
• NDIS Worker Orientation Module
• National Police Check (Aged Care/Vulnerable Persons – preferred)
• Current Australian driver's licence
Preferred:
• Triple COVID-19 vaccination + annual flu vaccine
• Current First Aid & CPR (within 12 months)
• Experience with wound and catheter care
