Care Partner – Right at Home Adelaide Region

Right at Home, Adelaide Region is seeking a qualified, reliable, and passionate Care Partner to join our growing team.

Key Responsibilities
  • Provide compassionate support through phone and online enquiries with timely, empathetic communication.
  • Meet with clients in their homes to support assessments and assist in developing tailored care plans aligned with Aged Care Quality Standards.
  • Manage client enquiries, feedback, service adjustments, and complaints with a solution-focused and person-centred approach.
  • Assist the Rostering team with master rosters and scheduling tasks to ensure efficient, high-quality service delivery.
  • Manage daily scheduling changes based on client needs and staff availability.
  • Support the care team through mentoring, training assistance, and performance monitoring.
  • Conduct staff observations and assist in credentialing care workers for relevant duties such as personal care and medication prompting (as appropriate).
  • Maintain accurate client and staff documentation in line with organisational policies and regulatory requirements.
  • Demonstrate adaptability and embrace challenges as opportunities for learning and growth.
  • Participate in a rotating on-call system for after-hours, weekend, and public holiday support (if required).

Working Hours

This role is Monday to Friday, predominantly 8:30am – 4:30pm (flexibility available).

Preferred Skills
  • 1–2 years' experience as a care worker, care partner, support worker, scheduler, case manager assistant, or similar role in Aged Care or Disability
  • Sound understanding of the Disability and/or Aged Care sectors
  • Strong organisational and time-management skills
  • Ability to work well under pressure and deliver an excellent standard of care
  • Strong relationship-building skills with diverse clients
  • Familiarity with Visual Care or similar Client Management Systems
  • Intermediate to advanced Microsoft Office skills (Excel, Word, Outlook)

Qualifications & Requirements
  • Certificate IV or V in Disability, Individual Support, or Aged Care (or equivalent)
  • Current National Police Check & Working with Children Check
  • Current First Aid and CPR certificate
  • Current driver's licence and reliable registered vehicle
  • Full comprehensive vehicle insurance
  • Excellent verbal and written communication skills
  • Smart mobile phone (iPhone or Android)
  • COVID-19 vaccination
  • Ability to commute or relocate to Prospect, SA 5082
  • Full Australian working rights (visa sponsorship considered for suitable applicants)
