We're looking for someone who's more than experienced – you're confident, resilient, and relentless about making sure clients get the best shot at independence, dignity, and choice.
What You'll Do- Develop, implement and regularly review individualised care plans of clients across disability, aged care or injury-recovery needs
 - Coordinate service requests with Care Coordinators to ensure timely, appropriate responses.
 - Lead the charge in developing and reviewing Behaviour Support Plans (BSPs) and strategies for clients with complex needs.
 - Navigate the tricky world of mental health and disability with compassion, creativity, and confidence.
 - Be the go-to advocate for clients and families – guiding them through NDIS pathways, SIL/SDA housing, and funding applications.
 - Train and coach support workers so every shift is safe, consistent, and client focused.
 - Spot risks before they escalate – because you know prevention is power.
 
- Background in mental health, disability, or behaviour support – you've done the hard yards and you're not afraid of complexity.
 - Strong grasp of NDIS frameworks and compliance requirements.
 - A calm but assertive presence when working with clients who may be distressed, aggressive, or at risk.
 - A natural problem-solver who can think on your feet and adapt fast.
 - Allied health, aged care, or disability qualifications (bonus points if you've worked across multiple funding streams).
