Care Partner

Country Mile Home Care is seeking a dedicated and proactive Care Partner to support and oversee our subcontracted service delivery teams across various regions. This role will play a pivotal part in maintaining strong relationships with our contractors, ensuring compliance with regulatory and internal standards, coordinating reimbursements for Aged Care Assistive Technology & Home Modifications (AT-HM), assist our compliance team for arranging relevant training, and streamlining communication between contractors and our compliance team.
Key Responsibilities
Contractor Relationship Management
Build and maintain positive working relationships with contractor companies.
- Serve as the first point of contact for contractor inquiries, grievances, or service-related clarifications.
Conduct regular check-ins with contractor teams to monitor performance and satisfaction.
AT-HM Reimbursement Coordination
Review, approve, and track reimbursements under the Aged Care AT-HM program in accordance with government guidelines.
- Liaise with clinical teams and contractors to verify justification and documentation of AT-HM claims.
Ensure that all claims are processed in a timely, accurate, and compliant manner.
Assist with Compliance Oversight
Assist compliance team to ensure contractors meet all mandatory compliance requirements, including police checks, NDIS worker screening, insurance, training, and incident reporting.
- Assist compliance team to maintain and audit contractor compliance records in collaboration with the internal compliance team.
Issue notices, reminders, and support where corrective actions are required.
Assist with Training Coordination
Identify training needs across contractor teams and coordinate appropriate internal or external training sessions (e.g. manual handling, clinical procedures, cultural awareness).
- Maintain training attendance and competency records.
Work closely with the quality and clinical teams to ensure training aligns with care standards.
Reporting & Communication
Provide regular updates to management on contractor performance, compliance status, and risk areas.
- Prepare quarterly summaries or dashboards on contractor engagement, AT-HM reimbursements, and training outcomes.
- Participate in internal quality improvement initiatives and meetings as required.
Key Requirements
- Previous experience in aged care, NDIS, or health sector contractor management (minimum 2 years preferred).
- Strong administrative and organisational skills, with the ability to handle multiple priorities.
- Excellent interpersonal and communication skills.
- High level of attention to detail and ability to interpret guidelines and funding criteria (e.g. AT-HM).
- Proficient in Microsoft Office.
Desirable
- Qualifications in Business, Community Services, Health Administration or similar.
- Chinese language speaking.
- Experience working with multicultural teams or subcontracted models of care.
