About the role:

The Social Support Group Program Coordinator leads the planning, delivery and ongoing development of the Social Support Program. The aim of the Program is to assist frail older people to participate in community life and feel socially included through structured, group-based activities that develop, maintain or support independent living and social interaction. Activities are conducted away from the participant's home and in, or from, a fixed base facility or community-based settings. The program is primarily funded by the Commonwealth Home Support Program (CHSP).

The Treehouse Program is a holistic outpatient service providing additional care options for people who are living with dementia.

This role would suit an experienced activities assistant, an enrolled nurse or someone with an interest in diversional therapy and activity programs.

Details of Appointment: 56 hours per fortnight

Salary: Subject to qualifications

Central Highlands Rural Health (CHRH) is a rural health organisation that operates five campuses across Hepburn Shire and the Macedon Ranges in the Central Highlands of Victoria in Australia, north and north-west of Melbourne. It was formed through the voluntary amalgamation of Hepburn Health Service and Kyneton District Health on the 30th November 2019. Each of our campuses retains the name of the township in which they are situated - Clunes Health, Creswick Health, Daylesford Health, Kyneton Health, and Trentham Health.
