An exciting opportunity exists for an Allied Health Care Coordinator (Level 3) with the Aboriginal Care Coordination Team (ACCT) at Sydney Children's Hospital Randwick. This is a temporary part-time job-share position (0.63 FTE) commencing January 2026 and ending 30 June

This role will provide care coordination for Aboriginal children and their families to manage their chronic conditions and link them into primary health care services. The role will work closely with an Aboriginal Health Worker and the La Perouse Aboriginal Clinic team, which is based with the Community Child Health team. The role is a joint initiative between The Sydney Children's Hospitals Network (SCHN) and Central and Eastern Sydney Primary Health Network (CESPHN) to increase care in the community for Aboriginal children.

The Care Coordinator will aim to support access, health promotion and preventative health services to Aboriginal communities living within the CESPHN catchment area. What you'll be doing

The primary focus of this position is for Aboriginal children and families to have access to high quality, evidence based and culturally safe community services to support optimal health, development and wellbeing. This role will work as a Care Coordinator in the Community Child Health team with Aboriginal children and their families to manage their chronic conditions and link them into primary health care services. The Community Child Health team offers comprehensive and accessible services, programs, and advocacy for Aboriginal children and their families in the La Perouse community plus further beyond The Sydney Children's Hospitals Network. The role will work as part of a multidisciplinary team.

The role includes clinical supervision with opportunities for further education and training. The role will work very closely with other members of the Aboriginal Chronic Care Team (ACCT) and the La Perouse Family Support Team (FST), which is based with the Community Child Health team. The role is a joint initiative between The Sydney Children's Hospitals Network (SCHN) and Central and Eastern Sydney Primary Health Network (CESPHN) to increase care in the community for Aboriginal children.
