About the Role

We are seeking a compassionate and dedicated Elders Care Support Worker to join our team on Darkinjung Country, supporting Aboriginal Elders to live well, stay connected, and maintain strong cultural and community ties.

This role focuses on providing personalised care coordination, access to health and wellbeing services, cultural engagement, and advocacy. You will walk alongside Elders to support their physical, social, emotional, and cultural wellbeing, ensuring they are empowered to live with dignity and independence.

You'll work collaboratively with families, health professionals, and community organisations to deliver wraparound, culturally safe care that respects Aboriginal values and community protocols.

Key Responsibilities

Provide individualised care and support to Aboriginal Elders based on their specific needs and preferences

Assist Elders in accessing health services, wellbeing programs, and community supports

Organise and facilitate cultural workshops and yarning circles that promote social connection, wellbeing, and cultural identity

Advocate on behalf of Elders to ensure their voices are heard and their rights are upheld within services and the community

Maintain accurate and confidential client records in line with organisational and sector standards

Work collaboratively with transdisciplinary teams to ensure holistic care and strong community outcomes

What You'll Bring

Knowledge and experience working with Aboriginal or Torres Strait Islander Elders and older people

Certificate III or IV in Aged Care (or equivalent qualification), or a minimum of two years' experience in the Aged Care or Community Services sector

Understanding of Aboriginal culture, values, and protocols, with a commitment to culturally safe and trauma-informed care

Strong communication and interpersonal skills to build trust and rapport with Elders, families, and service providers

Experience supporting social, emotional, and wellbeing needs, particularly in aged care or community contexts

Ability to work collaboratively within a team and maintain clear, professional boundaries
