The Role
As a Support Coordinator, you will play a pivotal role in providing exceptional support services to Level 2 and 3 participants.
Your primary responsibilities will include:
- Supporting participants to understand their NDIS plans and exercise choice and control.
- Connecting participants with supports, negotiating service agreements, and removing barriers.
- Assisting with plan reviews, reports, and achieving measurable outcomes.
- Building trust and maintaining continuity when stepping into existing caseloads.
- Collaborating with stakeholders to strengthen participant networks and resilience.
- Upholding Heartland values and person-centred practices.
What We're Looking For
- Relevant tertiary qualification (e.g., Social Work, Community Services) and minimum 2 years' experience in Support Coordination.
- Strong knowledge of the NDIS and disability support sector.
- Excellent communication, organisational and problem-solving skills.
- Ability to work independently while staying connected to a team.
- A genuine passion for making a difference in the lives of people with disability.
