Join Villa Maria Catholic Homes and lead a dedicated team delivering exceptional home care services to our community members. This full-time position offers the opportunity to make a meaningful difference in people's lives while advancing your leadership career in healthcare. We're seeking an experienced clinician with a strong background in Home Care Packages (HCP), Community Care, and Short-Term Restorative Care (STRC), who is passionate about supporting older Australians to live independently at home.

As our Clinical Team Leader, you'll guide and support our home care team in providing compassionate, person-centered care. You'll work closely with clients, families, and healthcare professionals to ensure the highest standards of care delivery while fostering a positive, collaborative team environment.

• Provide clinical leadership and mentorship to home care staff
• Conduct comprehensive client assessments and care planning
• Ensure compliance with regulatory standards and quality frameworks
• Collaborate with families, healthcare professionals, and multidisciplinary teams
• Support staff development through training and professional guidance
• Monitor and evaluate care outcomes to drive continuous improvement

About you

• Experience in home, community, and restorative care settings is essential. Experience solely in residential aged care is not sufficient
• Current AHPRA registration as a Registered Nurse (RN)
• Willingness to travel regionally once per fortnight
• Current Victorian driver's licence and National Police Check clearance.
• Proven experience in case management, assessment, care planning, and participant budget management.
• Strong written, verbal, and numerical communication skills & proficiency in MS Office.
• Knowledge of Aged Care Reforms, Quality Standards, and community aged care services.
