We are seeking a highly motivated and organized individual to join our team as a Full Support Coordinator/Psychology Recovery Coach. As a Support Coordinator, you will be responsible for coordinating and delivering support services to our clients. You will meet with referrals (clients and their stakeholders) to discuss their NDIS plan, their current supports and services, how they are using the plan, their goals, and any other relevant information. Your role will include analysing clients' plans to understand how to implement them, sourcing appropriate supports, promoting quality and cost-effective interventions and outcomes, and providing appropriate information to clients. You will need strong verbal and written communication skills to work with clients, their stakeholders, therapists, support providers, funding bodies and others daily.
Key Responsibilities:
- Work closely with NDIS participants to understand their goals and develop individualised support plans
- Coordinate and monitor the delivery of services and supports in line with participants' plans.
- Assist participants in connecting with providers, community services, and resources.
- Provide Psychosocial recovery coaching to individuals with Mental health conditions - focusing on building resilience, goal setting and enhancing life skills
- Provide guidance on NDIS funding and assist in budgeting and plan management.
- Advocate for participants to ensure their needs are met and that they receive the appropriate support.
- Maintain accurate records and documentation in accordance with NDIS guidelines.
- Provide regular updates and reports to participants, families, and relevant stakeholders.
- Collaborate with other healthcare professionals, service providers and community resources to create a hollistic support network for participants.
Key Skills and Experience:
- Previous experience as a Support Coordinator, Psychology Recovery Coach or in a similar role within the NDIS sector.
- Knowledge of the NDIS framework and an understanding of NDIS planning processes.
- Excellent communication skills, with the ability to build rapport with clients, families, and service providers.
- Strong organisational and time-management skills, with the ability to manage multiple cases simultaneously.
- A compassionate, patient, and solution-focused approach.
Qualifications:
- Relevant Qualification in Disability, Social Work, Community Services, or related qualification.
