About the role

As an Experienced Support Coordinator at Personal Support Solutions, you will play a pivotal role in empowering individuals with disabilities to achieve their goals and live more independent lives. This full-time position is based in Mittagong & Goulburn, NSW and involves working closely with participants to coordinate and facilitate the delivery of their NDIS supports.

What you'll be doing

  • Developing personalised support plans that align with participants' NDIS goals and budgets
  • Coordinating and monitoring the delivery of supports, ensuring they are meeting participants' needs
  • Assisting participants to access the services and equipment they require
  • Building and maintaining strong relationships with service providers, Allied Health professionals and the NDIA
  • Providing guidance and assistance to participants and their families to help them navigate the NDIS system
  • Regularly reviewing and updating support plans to adapt to participants' changing needs
  • Documenting all interactions and maintaining accurate records in line with NDIS requirements

What we're looking for

  • Tertiary qualifications in a disability, community services or allied health field
  • Experience Support Coordinator with a strong understanding of the NDIS and its processes
  • Excellent communication and interpersonal skills, with the ability to empathies and build trust with diverse clients
  • Strong organisational and time management skills, with the ability to prioritise and multitask
  • A genuine passion for supporting people with disabilities to achieve their goals and live more independent lives
  • A collaborative mindset and the ability to work effectively as part of a team
  • A current driver's licence and access to a reliable vehicle
