About the Role:

  • Max-term Part-time role, providing specialised, evidence-based healthcare and care coordination to adults experiencing secondary or tertiary homelessness across the City of Sydney Local Government Area. Delivers culturally competent, person-centred, trauma-informed care that addresses complex health and social support needs in alignment with the Homeless Health Service values and recovery-oriented practice principles.

  • Engage clients in comprehensive health assessments and care planning, providing clinical interventions and referrals to improve access to mainstream and specialist healthcare, social supports, and housing services. Supports clients in navigating barriers to care and achieving improved holistic health outcomes.

  • Establish and maintain collaborative partnerships with clients, their cultural and social supports, local service providers, and communities. Develop multi-agency care plans, coordinate case conferences, and strengthen client support networks to facilitate pathways out of homelessness.

  • Lead coordination of care across multiple healthcare and social contexts, ensuring safe, effective, and culturally responsive interventions. Participate in multidisciplinary clinical review meetings, provide discharge planning, and liaise with internal and external stakeholders to optimise client outcomes.

  • Deliver education, supervision, and capacity building, providing clinical guidance, consultation, and support to staff, students, and external partners. Actively contribute to service development, quality improvement initiatives, and evaluation processes to enhance client care.

  • Maintain professional standards, accountability, and ongoing development, including participation in clinical supervision and continuing professional development activities. Contribute to reporting, research, and policy initiatives that advance the Homeless Health Service and improve care for people experiencing homelessness.

About You:

  • Degree in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW), with current evidence of Level 2 (or higher) practice and extensive experience in supporting clients with complex and co-occurring health and social needs.

  • Demonstrated expertise in autonomous, culturally competent, and trauma-informed clinical casework, delivering safe, strengths-based interventions for people experiencing homelessness and associated health and social challenges.

  • Strong understanding of homelessness and related systemic issues, with a commitment to trauma-informed care, recovery-oriented practice, and addressing barriers to health and social service access for diverse client populations.

  • Proven ability to work at a senior level within interdisciplinary teams, contributing to service development, education, and quality improvement initiatives, while fostering collaborative relationships across organisational and community stakeholders.

  • Committed to ongoing professional development and clinical supervision, with a track record of reflective practice, mentoring, and supporting colleagues in building capability and enhancing client care.

  • Highly developed communication and digital literacy skills, with a demonstrated capacity to engage constructively with clients, colleagues, and external partners, complemented by a current NSW Driver's License to support community-based work.
