At Tender Loving Care Aged Care, we are passionate about supporting older people to live with dignity, independence, and choice. With aged care reforms commencing from 1 July, this is an exciting time to join our team as we expand and strengthen our Support at Home services.

We are seeking a Support at Home Coordinator to play a pivotal role in delivering high-quality, person-centred support across Home Care Packages (HCP), the new Support at Home program, and complex Commonwealth Home Support Programme (CHSP) clients.

About the Role

As a Support at Home Coordinator, you will:

  • Act as the primary point of contact for clients and families in your region
  • Develop and review care plans that reflect client goals and clinical recommendations
  • Coordinate onboarding, reassessments, and case conferences
  • Work closely with intake, operations, and allied health professionals to ensure smooth service delivery
  • Ensure compliance with the Aged Care Act 2024 and Aged Care Quality Standards
  • Maximise client funding and apply for supplements where appropriate
  • Promote our services through community engagement events and partnerships
About You

You are an experienced care coordinator who thrives on supporting people to live well at home. You will bring:

  • Minimum Certificate IV in Aged Care, Community Services, or similar (Diploma preferred)
  • Experience with HCP, CHSP, or Support at Home models
  • Strong communication and organisational skills
  • Ability to manage competing priorities and a diverse caseload
  • Working knowledge of aged care reforms and quality standards
  • Proficiency in client management systems and digital tools
  • A current driver's licence and reliable vehicle
  • NDIS Worker Screening Check & Police Check (or willingness to obtain)
