About the role:
As an Intake Clinician in the Step Thru Care program, you'll be the crucial first connection for individuals seeking support for mental health and alcohol and other drug (AOD) concerns. Every conversation you have, every assessment you complete, and every plan you coordinate has the potential to shift someone's life trajectory. You'll be responsible for conducting initial intake and risk assessments, coordinating with internal and external service providers, and ensuring smooth transitions into appropriate supports. Working within a passionate, multi-agency team, you'll play a key role in ensuring people feel heard, supported, and guided from their very first contact.
What you will bring to the team:
Excellent communication and interpersonal abilities, with a warm, non-judgemental approach and the capacity to manage sensitive or challenging situations.
A collaborative mindset, with demonstrated experience working in multi-disciplinary teams and across multiple organisations.
Knowledge of the community health system, including local service providers, stepped care models, and a commitment to the social model of health.
Organised and adaptable, able to prioritise consumer needs, maintain accurate documentation, and meet reporting and performance requirements.
