About the Role

As a Support Coordinator, you will play a vital role in helping our participants break through barriers and actively participate in their local communities.

Key responsibilities:

  • Implementing, connecting, and coordinating supports.
  • Making effective referrals to preferred services.
  • Providing guidance for NDIS plan reviews and change of circumstances.
  • Supporting participants during crisis and complex situations.
  • Collaborating with various mainstream and community services.
  • Helping participants build capacity and develop meaningful relationships.

About You Qualifications, skills, and experience:

  • Extensive experience in NDIS Support Coordination / Case Management (3–10+ years).
  • Tertiary qualification in Allied Health, Social Work, Counselling, Nursing, or Human Services (or significant NDIS Support Coordination experience).
  • Proven ability to work both independently and collaboratively in a team.
  • Strong rapport-building skills with diverse customers.
  • Expertise in supporting customers with complex needs.
  • Energetic, driven, and committed.
  • Excellent communication, administrative, and documentation skills, including proficiency with online portals and databases.
  • Established connections with disability, mainstream, and community networks.
  • Friendly and positive personality, open and adaptable to innovation.
  • Valid driver's license (essential).
  • Mental health expertise (desirable).
  • Experience with acquired brain injury or spinal injury (desirable).
  • Current Working with Children Check and Police Check (essential or willing to obtain).
  • NDIS Worker Screening and Orientation (essential or willing to obtain).
