Primary objective

Deliver high quality, culturally appropriate and effective services within the Front Door Frailty Program based in the Emergency

Department.

Context statement

The Front Door Frailty (FDF) service is a new initiative funded by the Commonwealth Government under Strengthening Medicare. It aims

to improve care and support for older Territorians living with frailty, enabling individuals to remain living at home and community safely.

The FDF program will operate across Royal Darwin Hospital Emergency Department and the new Community Frailty Service, based at

Casuarina Plaza. Allied Health positions within the FDF team at Royal Darwin Hospital Services will provide timely assessment and

intervention for individuals with frailty, aiming to prevent hospital admission and safe and rapid discharge home. Service delivery will be

provided by multidisciplinary team based in the Emergency Department, working closely with the Community Frailty Service.

Key duties and responsibilities

1. Provide timely, clinically and culturally appropriate occupational therapy services under minimal supervision within a multidisciplinary

framework.

2. Provide feedback to senior staff and management on program and clinical issues.

  1. Provide clinical/professional supervision and/or support to students and relevant staff
  2. Prioritise and manage a caseload within a dynamic and evolving work environment to ensure timelines and health outcomes are met.
  3. Co-ordinate and/or contribute to continuous quality improvement and service development activities at a local or regional level to

ensure delivery of contemporary [insert profession] services.

6. Contribute to the achievement of work unit goals and key performance indicators.

  1. Follow defined service quality standards, work health and safety policies and procedures relating to the work being undertaken in

order to ensure high quality, safe services and workplaces.

8. Foster a positive workplace culture through exemplary leadership practices and role modelling behaviour.

Selection criteria

Essential

1. Relevant qualifications in Occupational Therapy that meets membership eligibility requirements of Occupational Therapy Australia

and registration for practice with the Australian Health Practitioners Regulation Authority.

2. Post graduate experience in providing contemporary high level clinical occupational therapy services to patients and families/carers.

3. Demonstrated awareness and understanding of Aboriginal culture and proven experience in working effectively with patients and

families/carers of diverse cultural and social backgrounds.

4. Well-developed communication, interpersonal skills, negotiation and conflict management skills, with experience working in a team

environment to achieve positive health outcomes.

  1. Demonstrated ability to provide clinical supervision of students and/or staff and a commitment to professional development.
  2. Proven ability to implement and complete continuous quality improvement and service development activities.

  3. Proven ability to efficiently and effectively manage a high volume workload.

Desirable

1. Demonstrated experience providing evidence based allied health services to people with dementia, ageing and frailty.

2. Post-graduate qualifications or experience in clinical education, leadership / management or speciality practice area.
