Latrobe Regional Health (LRH) currently has HIP Coordinator position available to join our growing team on a part-time basis 0.4 EFT, fixed term until 24 May 2026.
The key responsibilities of the Health Independence Program (HIP) Coordinator to contribute to the continuum of care with the aim of improving individual health outcomes for patients requiring support post discharge from Latrobe Regional Health. The primary purpose of the Care Coordinator role is to assess individual client needs and manage a case load of clients accepted and enrolled in one or more of the Health Independence Programs available at LRH.
About you
Ideal candidates for this role will have:
- Current Australian Health Practitioner Regulation Agency (AHPRA) registration as Registered Nurse.
- Minimum 5 years' experience post-graduate experience in acute nursing field.
- Appropriate skills and experience in patient management.
- Knowledge of relevant evidenced based disease management guidelines.
- Critical thinking and problem-solving skills.
- Post graduate education in a relevant field - eg cardiac, respiratory (desirable).
