About the Role
As a Support at Home Team Leader, you will lead and inspire a team of Care Partners to deliver safe, effective, and client-centred care to the ageing and elderly multicultural community. As a team leader of 12 staff, you will champion best business practice in service delivery, quality care, and compliance ensuring that our support and service meets both our clients expectations, and the requirements of the strengthened standards.
This role is a unique blend of leadership, coaching, mentoring, client relationship management, and compliance oversight. You will play a critical part in building a culture of accountability, continuous improvement, and shared purpose.
Key Responsibilities
· Lead, mentor and support a team of Care Partners to deliver safe, person-centred, and compliant home support.
· Monitor and uphold compliance with the Aged Care Strengthened Quality Standards, Support at Home reforms, and organisational policies.
· Oversee incident reporting, complaints management, and risk identification to ensure timely and appropriate resolution.
· Facilitate regular supervision, performance reviews, and ongoing capability development.
· Review and support the delivery of care aligned with individual client goals and regulatory frameworks.
· Collaborate with internal teams (finance, clinical, rostering, intake) to streamline service coordination.
· Audit service documentation, case notes, and support plans to ensure accuracy and quality.
· Collaborate with internal and external stakeholders to coordinate integrated, holistic care.
· Drive quality improvement initiatives and actively contribute to accreditation readiness.
About You
You're a natural leader with a strong background in aged care or community services. You thrive in a dynamic environment, can juggle multiple priorities, and are passionate about helping people live independently.
To be successful, you will have:
· Obtained work experience within the aged care sector.
· Demonstrated experience in team leadership and people management
· Strong understanding of the Aged Care Quality Standards and person-centred care
· Excellent communication and interpersonal skills
· High-level organisational and problem-solving skills
· Qualifications in either Aged Care, Community Services, Nursing is desirable
· The ability to speak a second language is highly regarded.
· Current Police Check and relevant clearances (or willingness to obtain)
