About the Role

At ConneXtions, our Support Workers direct participant support in line with our Values and Vision, by walking alongside participants in their individual journeys, empowering and supporting capacity building outcomes to assist participants to reach personal goals and outcomes, whilst ensuring their active participation as valued member of the communities in which they live.

We are currently compassionate, reliable Disability Support Workers to deliver person-centred support at home within a structured care plan and predictable roster.

Day to day responsibilities and duties of the position may include:

  • Personal care (showering, dressing, grooming)
  • Hoist & manual handling (safe transfers, repositioning)
  • Bowel care and continence support (as per plan)
  • Mealtime support & meal preparation (incl. modified diets if required)
  • Accurate shift notes, incident reporting, and compliance with NDIS Practice Standards

Roster: 24/7 supports including day/evening, inactive sleepover nights (8 hours), weekends & public holidays

Typical shift pattern (daily)

  • Shift 1 (A): 20:00–09:00 — includes inactive sleepover 21:00–05:00 (active 20:00–21:00 & 05:00–09:00)
  • Shift 2 (B): 08:00–15:30
  • Shift 3 (C): 14:30–21:00
  • Double-staffed/2:1 repositioning times (daily): 08:00–09:00, 14:30–15:30, 20:00–21:00

About You

If you are seeking a role that offers meaning and purpose, and have skills and experience in direct support work, we would love to hear from you

We are looking for people that have a minimum Certificate III Individual Support or Certificate IV in Disability and a willingness to complete training as requested.

You must be available to do a variety of shifts, including early mornings, overnight and awake shifts

You will be driven by a desire to help our participants achieve their goals and be relatable to the people we are supporting.

You will be passionate about having a positive impact and supporting people with disabilities to reach their full potential.

As a condition of employment, you are required to obtain and maintain compliance with:

  • NDIS Worker Screening Check
  • NDIS Worker Orientation Modules
  • Working With Children Check (VIC & NSW)
  • Current First Aid Certificate including CPR.
  • Current driver's licence & access to a registered vehicle with Comprehensive Insurance
  • Mandatory online training
