Position: Care Coordinator

Location: Stones Corner Office, Brisbane

Responsibilities:

  • Build strong relationships with clients and care workers, ensuring seamless communication and exceptional service.
  • Manage a caseload of up to 40 clients living with various disabilities, coordinating their care plans, budget management, and promoting independence.
  • Conduct home visits to introduce clients to our compassionate care workers and provide ongoing support.
  • Utilise computer skills for efficient record-keeping and communication.

Requirements:

  • Minimum of 18+ months of experience in the Aged and Disability Sector as an Enrolled Nurse
  • Valid QLD Driver's License.
  • First Aid & CPR Certificate.
  • Yellow Card/NDIS Worker Screening Clearance (Paid).
  • Federal Police Check.
  • Strong computer literacy.

Schedule:

  • 8 hour shift
  • Monday to Friday
  • On call
  • Overtime

Ability to commute/relocate:

  • STONES CORNER, QLD 4120: Reliably commute or planning to relocate before starting work (Preferred)
