**Position**: Care Coordinator

**Location**:Stones Corner Office, Brisbane

**Responsibilities**:

- Build strong relationships with clients and care workers, ensuring seamless communication and exceptional service.
- Manage a caseload of up to 30 clients living with various conditions, coordinating care plans, budget management, and promoting independence.
- Conduct home visits to introduce clients to our compassionate care workers and provide ongoing support.
- Utilise computer skills for efficient record-keeping and communication.
- Answer calls from clients and care workers.

**Requirements**:

- Minimum of 12+ months of experience in the Aged and Disability Sector, preferably as a care worker or similar role.
- Valid QLD Driver's License.
- First Aid & CPR Certificate.
- COVID19 Vaccinated
- Yellow Card/NDIS Worker Screening Clearance (Paid).
- Federal Police Check.
- Strong computer literacy.

**Salary**: $60,000.00 - $65,000.00 per year

**Benefits**:

Schedule:

- 8 hour shift
- Monday to Friday
