Mulungu Aboriginal Corporation Primary Health Care Service is committed to enhancing the lives of Aboriginal and Torres Strait Islander people through comprehensive primary health care. Our organization operates as a community-controlled entity, delivering a range of health and social services tailored to meet the unique needs of our community in Mareeba, Tablelands, and surrounding areas.
With a strong focus on social and emotional well-being, the Culture Care Connect Team is an integral part of our Social Services division, working collaboratively to ensure culturally appropriate support and care for our clients.
Your Role as an Aftercare WorkerAs the Culture Care Connect Aftercare Worker, you will play a crucial role in providing timely and culturally sensitive follow-up care for Aboriginal and Torres Strait Islander individuals who have experienced a suicide attempt or crisis. You will facilitate care coordination that empowers individuals, promoting resilience and a holistic sense of safety and wellness.
In this dynamic position, you will collaborate closely with a diverse range of stakeholders to build strong referral pathways and ensure effective communication within the community. Your contributions will help shape our localized service delivery model, ensuring it meets the specific needs of those we serve.
Who We're Looking ForWe are seeking an enthusiastic and compassionate individual who shares our commitment to improving the lives of Aboriginal and Torres Strait Islander peoples.
- A Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care or equivalent experience.
- Experience in mental health support or related fields.
- Exceptional communication skills and a strong understanding of the cultural factors affecting our community.
- A proactive approach to case management and crisis intervention.
Your knowledge of Aboriginal and Torres Strait Islander cultures, coupled with your ability to work independently and as part of a multi-disciplinary team, will be essential in delivering outstanding care and support to our clients.
Selection Criteria
SC 1 Demonstrated knowledge and understanding of Aboriginal and Torres Strait Islander cultures and the social, economic, and cultural factors affecting Aboriginal and Torres Strait Islander people in today's society.
SC 2 Demonstrated thorough knowledge and understanding of SEWB and Mental Health frameworks as well as best practice guidelines and treatment options for suicide prevention and self-harm risk.
SC 3 Demonstrated sound interpersonal and communication skills including the ability liaise and network with a range of stakeholders including internal and external service providers as well as effectively communicate with Aboriginal and Torres Strait Islander people of all ages.
SC 4 Ability to work both in a multi-disciplinary team environment, independently with limited supervision, prioritise and meet deadlines, manage matters of a sensitive and confidential nature and meet organisational demands.
SC 5 Demonstrated ability to implement and work within a case management framework to deliver services including the ability to undertake crisis intervention and work with individuals referred to the service.
