Key Responsibilities
- Develop, implement, monitor, and review individualised care plans to ensure person-centred, goal-directed service delivery.
- Update care plans and budgets to reflect changes in clients' health, needs, and preferences while ensuring accurate and compliant documentation.
- Ensure compliance with NDIS Commission Standards, TAC, Aged Care Quality Standards, Department of Veterans' Affairs (DVA) requirements, and relevant legislation.
- Maintain high-quality, unbiased documentation of all client interactions and stakeholder communications.
- Conduct WHS assessments in client environments and apply appropriate risk management strategies to ensure safety.
- Lead holistic case management practices including goal setting, care worker introductions, client supervision, and service monitoring.
- Act as the primary point of contact for clients regarding service delivery, incidents, and support, handling inbound and outbound communication.
- Respond to client feedback, complaints, and incidents promptly to foster a culture of continuous improvement.
- Collaborate with multidisciplinary teams including GPs, allied health professionals, welfare agencies, DVA, and community organisations.
- Refer clients to appropriate healthcare professionals and third-party services in alignment with care goals and funding provisions.
- Promote culturally responsive care planning and support services, particularly for clients with special or diverse needs.
- Escalate clinical concerns appropriately and ensure clients receive guidance from registered clinical professionals when needed.
- Support care staff with clear scopes of practice and provide individualised guidance based on client care plans and goals.
- Manage service delivery in line with allocated funding and budgets, resolving financial queries and maintaining cost-effective operations.
- Promote effective teamwork and communication across staff, clients, families, and service partners to achieve optimal care outcomes.
- Prepare professional reports and documentation for internal use, external stakeholders, and funding bodies.
Qualifications and Skills
- Registered Nurse with Mental Health experience OR Aged Care (Nurse-In- Charge)/ Clinical Manager experience.
- Comfortable working and supporting clients in their own homes.
- Full Driving Licence with insured vehicle.
- Proven expertise in developing and managing individualised care plans aligned with client needs and goals.
- Thorough understanding of NDIS and Aged Care Quality Standards.
- Competent in WHS assessment and implementation of safety protocols in client settings.
- Highly skilled in accurate, professional client documentation and case notes.
- Strong ability to collaborate with multidisciplinary teams and coordinate referrals.
- Experienced in budget oversight and navigating multiple funding models for client care.
- Organised and efficient with excellent time management across office and community environments.
- Proficient in computer systems and analytical reporting tools.
- Deep commitment to the health, dignity, and well-being of seniors and vulnerable individuals.
- Culturally aware and inclusive in-service planning and delivery.
- Management of AFTER-HOURS phone calls.
Job Types: Full-time, Part-time
Pay: $50.00 – $60.00 per hour
