STAR Community Services is expanding into Support at Home and we're seeking dedicated Physiotherapists and Occupational Therapists to join our team as Clinical Care Partners.
Who is STAR Community Services?
STAR Community Services is an award-winning not-for-profit supporting elderly and people with disabilities since 1996. We provide affordable, reliable aged care and disability services—including Support at Home, NDIS, transport, social activities, and companionship—to help clients live independently and stay connected. With help from dedicated staff and local volunteers, we serve Redlands, Ipswich, Logan, Gold Coast, and Toowoomba. STAR is committed to building inclusive communities and combating social isolation through regular programs and events.
About the Role (not limited to)
As a Clinical Care Partner, you'll play a vital role in our multidisciplinary team, delivering restorative and preventative health interventions that support older people to maintain or regain their physical, functional, and cognitive abilities. You will:
- Conduct comprehensive clinical assessments and design evidence-based therapy plans.
- Deliver strength-building, mobility, and falls-prevention programs.
- Train and empower participants (and carers) with self-management strategies.
- Prescribe assistive technology and recommend home modifications.
- Collaborate closely with Care Partners, nursing staff, and support workers to ensure holistic, person-centred care.
- Document progress and contribute to ongoing evaluation and quality improvement.
What We're Looking For
- Degree in Physiotherapy or Occupational Therapy with current AHPRA registration.
- Experience in aged care, community care, or home-based allied health services.
- Strong understanding of restorative care, reablement, and falls prevention.
- Ability to prescribe assistive technology and support home safety adjustments.
- Excellent communication, collaboration, and documentation skills.
- Empathy, adaptability, and a genuine passion for improving the lives of older people.
Desirable: Experience with CALD communities, dementia care, or trauma-informed practice.
