NDIS Case Manager

We support people with disability and the elderly across a range of settings, including but not limited to daily living, transport, domestic assistance, personal care and community participation. As part of our team you'll help shape meaningful outcomes, build capacity, and empower clients to exercise choice and control in their lives.
The RoleAs a Case Manager at EnableU you will play a pivotal role in supporting NDIS participants to achieve their goals. You will assess needs, coordinate supports, manage service delivery, monitor progress and ensure participants receive consistent, high-quality support aligned with their NDIS plan and preferences.
Key Responsibilities- Conduct intake, assessment and develop individualised support plans for participants.
- Coordinate and manage supports, providers and services in line with the participant's NDIS plan and desired outcomes.
- Liaise with participants, families/carers, allied health professionals and external service providers to ensure integrated, seamless support.
- Monitor, review and document progress against goals, outcomes and service delivery indicators.
- Ensure all service delivery complies with NDIS regulations, EnableU policies and funding constraints.
- Advocate on behalf of participants to ensure their voice, preferences and aspirations are central.
- Foster community connection and participation, enabling greater independence and choice.
- Maintain accurate case records, incident reports, service agreements and other required documentation.
- Build and maintain strong relationships with stakeholders: participants, families, support staff, therapists, partner agencies.
- Contribute to continuous improvement initiatives, training and policy development as required.
- Previous experience working in the NDIS space (case management, support coordination or equivalent).
- Strong understanding of NDIS funding models, plan management and service delivery frameworks.
- Excellent communication (verbal and written) and interpersonal skills, especially in building trusting relationships with participants and stakeholders.
- Ability to manage multiple participants and priorities, with strong organisational and time-management skills.
- Commitment to person-centred, strengths-based practice.
- Current Australian driver's licence and access to a vehicle (if required for home/community visits).
- Current NDIS Worker Screening Check, or willing to obtain prior to commencement.
- Tertiary qualification or equivalent experience in a relevant field (e.g., Disability, Social Work, Community Services, Health) is highly desirable.
