About the role

  • We are seeking 2 Registered Nurses – one based north of the river and one south of the river.
  • Work closely with our local teams to coordinate and deliver high-quality in-home care.
  • Conduct clinical assessments and provide clinical care services.
  • Develop care plans, in consultation with customers and local teams, that reflect individual goals and needs.
  • Communicate effectively with local teams and others involved in the customer's care.
  • Monitor quality, safety, and compliance with clinical standards and regulatory requirements.
  • Use technology and digital systems to streamline processes and maintain accurate, up-to-date records.

About You

You are a qualified Registered Nurse with a passion for community care and a commitment to supporting people to live independently. 

  • Demonstrated experience in in-home aged and community care
  • Strong clinical assessment and care planning skills.
  • Excellent coordination and communication abilities.
  • Confidence in using digital platforms and technology to support efficient care delivery.
  • A collaborative, customer-centred approach with a focus on continuous improvement.

  • Registered Nurse with AHPRA registration
  • Experienced in community or home-based care
  • Valid driver's license, your own vehicle available to use for work related travel and love of building relationships in teams and the community Genuine belief that everyone deserves to belong
