We are looking for experienced Disability Support Workers with care coordination experience to join our amazing team This dual role offers a mix of office-based coordination and community support work across the Sutherland Shire and St George area.

About the Role

This is a dual-position role combining Care Coordination and Disability Support Work.

As a Care Coordinator, you will:

  • Organise, monitor and manage clients' care to ensure they receive the right supports at the right time
  • Create or update support plans and service agreements
  • Be the main point of contact for clients, families, and service providers
  • Help ensure smooth, consistent, and high-quality service delivery

As a Disability Support Worker, you will:

  • Provide domestic assistance, personal care, and medication administration
  • Support participants with community access, social activities and goal achievement
  • Promote independence, wellbeing, and positive outcomes
  • Make a meaningful difference in the lives of people living with disabilities

To Be Successful, You Will Need

  • Minimum 1+ years experience in Disability Support Work and Care Coordination
  • Experience in aged care, nursing, or nursing studies (highly advantageous)
  • Availability to work Monday to Sunday, with flexibility for multiple shifts
  • Full availability preferred to support comprehensive coverage
  • Current WWCC and NDISWC (required prior to applying)
  • Current First Aid & CPR (required before commencement)
  • Driver's licence, reliable vehicle, and comprehensive insurance
  • Strong written and verbal communication skills
  • Warm, nurturing, and compassionate nature
  • Local applicants from Sutherland Shire, St George or surrounding suburbs highly regarded
