We are establishing a Day Centre in Palmerston to provide a safe, inclusive, and culturally appropriate space for people experiencing homelessness or social disconnection. The centre offers practical support — showers, laundry, meals, lockers, and digital access — and tailored case management through a wraparound, trauma-informed approach.
The Opportunity
As a Key Worker, you will play a vital role in providing person-centred support to individuals and families accessing the Day Centre. You'll conduct triage, assessment, case management, and supported referrals to housing, health, and community services — promoting dignity, stability, and independence for clients.
Key Responsibilities
Conduct client intake, screening, and needs assessments.
Deliver culturally safe case management and goal-oriented support plans.
Provide supported referrals to housing, health, legal, and wellbeing services.
Assist clients in accessing practical services including hygiene, meals, and digital access.
Build strong relationships with partners such as Lutheran Care, Larrakia Nation, and other service providers.
Maintain accurate case notes and ensure compliance with Yilli Housing and NRSCH standards.
About You
You're passionate about community wellbeing and have experience supporting people who are experiencing homelessness or vulnerability. You bring empathy, cultural awareness, and a solutions-focused mindset.
Essential:
Experience in community services, housing, or a related sector.
Strong understanding of trauma-informed and culturally safe practice.
Excellent communication, de-escalation, and problem-solving skills.
Knowledge of local referral pathways (housing, AOD, mental health, financial).
Administrative and IT competency for reporting and case management.
Desirable:
Qualification in Community Services or similar field.
Lived experience or understanding of homelessness.
First Aid, Mental Health First Aid, or AOD training.
