Position summary
The Pathways to Good Health initiative is funded by the Victorian Government and provides
health screening, assessment, referrals, and health planning for Victorian children aged 0–
17 years entering, or recently in, statutory care. This includes children and young people in
home-based care and residential care.
The program is led by Grampians Health and delivered across the Wimmera and Northern
Grampians shires through a multidisciplinary model involving a Multidisciplinary team
incorporating a Grampians Community Health program Coordinator and Allied Health
clinicians, with close links to Paediatric outpatient clinics. Grampians Community Health
(GCH) has been funded to provide the Navigator role, supporting the care and coordination
needs of children and families as part of this collaborative model.
This role will support children and young people entering statutory care to access health
assessments and care planning by providing culturally responsive, child-centred
coordination and advocacy. The Navigator will help ensure that each child's health needs
are clearly identified and communicated, while supporting families, carers, and services to
understand and respond to these needs.
Work Location
The position will be based across both the Child Protection office (Horsham) and the
Grampians Community Health office. These days will be determined by service needs and
operational requirements.

Key responsibilities
1. Provide tailored, child-centred navigation for children aged 0–17 entering, or recently
entered into statutory care, supporting access to assessments, follow-up, and health
planning.
- Stakeholder Collaboration:Work closely with Grampians Health, Area Child Protection,
ACCHO's and Family Services Organisations to facilitate timely referrals and coordinated
care.
- Advocacy & Engagement: Ensure the voices of children, young people, families, carers,
and cultural supports are heard and incorporated in care planning and service delivery.
- Care coordination and assist in tracking and following up on care pathways and health
recommendations, ensuring communication flows between relevant stakeholders.
- Maintain accurate records of Navigator activity and contribute to required program
reporting in collaboration with the Grampians Health project team.
- Work in culturally safe and inclusive ways, particularly in supporting Aboriginal children,
families, and communities.
Provide supervision and support to students as required.
Foster a respectful and inclusive work environment by treating colleagues, clients and
other stakeholders with courtesy and respect.
- Other duties commensurate with current skills and experience as agreed with the
Program Leader, Manager, General Manager or the Chief Executive Officer.
Key selection criteria
Qualifications
• Tertiary qualification in Social Work and eligibility for AASW membership
- Experience working with vulnerable children and families, especially within statutory or
family services context
Mandatory
• Ability to support and advocate for children and carers navigating complex systems
- Understanding of trauma-informed and culturally safe practice
Desirable
• Proven ability to work collaboratively with government and community-based
organisations
- Strong understanding of and commitment to working respectfully with Aboriginal children
and families
- Demonstrated attention to detail with well-developed administrative and organisational
skills to effectively manage high volumes of work and determine priorities, meet targets
and deadlines.
- High level verbal and written communication skills that enable effective and appropriate
communication with a broad range of people at all levels.
Demonstrated skills and experience
• Ability to maintain confidentiality at all times.
- Demonstrated knowledge and application of computer software, including Microsoft
Office and the Internet.
- Demonstrated ability to contribute to positive workplace culture and practices.

Licences and registrations
• Current Victorian driver licence

Conditions of employment
This position is fixed term ceasing on or before 30 June 2027 and is subject to:
- Successful completion of a six-month probationary period

employment at Grampians Community Health. Where check results are unsatisfactory in
relation to the role to be carried out, the offer of employment will be withdrawn.
