The Clinical Nurse Consultant Nurse Navigator, Charleville, Western Qld, utilises advanced clinical skills and professional decision making to deliver a safe and efficient patient journey across the care continuum through the reduction of service fragmentation, educating and empowering individuals and coordinating patient care in a positive way.
