About the role

In this role, you'll deliver high-quality, person-centred care to individuals with diverse disabilities empowering independence and supporting clients to achieve their personal goals.

We offer set rosters with flexible shifts to support work–life balance:

In-Home Care: Shifts between 6am–10pm, including Active & Non-Active Overnights

Community Access: 8am–3pm, inclusive of set alternating weekends for diversity in shared rosters

What you'll be doing

  • Assisting clients with daily living activities such as personal care, hoisting, manual handling, meal preparation, and household tasks
  • Facilitating access to community resources and supporting social and recreational participation
  • Implementing individualised support plans and collaborating with clients, families, and other professionals
  • Advocating for client rights and ensuring a safe, inclusive, and empowering environment
  • Documenting client progress and maintaining accurate records
  • Participating in regular training and development to enhance your skills and knowledge
  • High complex skills desirable assisting in nursing cares, IDC/SPC catheter management, bowel cares (enema insertion) and wound cares under clinical nursing care plans.

What we're looking for

  • A Certificate III or IV in Disability Support or a related field - 2 years field Exp.
  • Demonstrated experience in providing person-centred care and support to individuals with disabilities
  • Exceptional communication and interpersonal skills, with the ability to build trust and rapport
  • A compassionate, patient, and empathetic approach, with a genuine commitment to making a difference
  • Flexibility to work a casual yet set roster schedule, including weekends and public holidays
  • A valid driver's licence and access to reliable transportation
