About the role…

The Restorative Care Partner is responsible for the clinical coordination and delivery of restorative and rehabilitative care services in accordance with a client's support plan. The Restorative Care Partner ensures optimal health outcomes for clients while promoting independence and functional recovery, and ensures culturally responsive, holistic, and inclusive care is provided.

This full-time, permanent position is based at our Bella Vista, NSW office, with hybrid working arrangements available.

The key aspects of this role will include:

  • Community & stakeholder engagement
  • Assessment & restorative care coordination
  • Ongoing support, advice & guidance
  • Quality care processes & risk management

About You

To be successful in this role you will have:

  • Current registration as a Registered Nurse or Allied Health professional
  • Specialist certifications / qualifications in relevant disciplines are highly regarded
  • Extensive experience in clinical coordination, case management, or restorative care within aged care, disability, or community services.
  • Strong understanding of reablement, functional rehabilitation, and person-centred care models.
  • Proficiency in risk management, compliance, and quality assurance processes.
  • High-level communication, problem-solving, and decision-making abilities.
  • Competence in using clinical documentation and care management systems.
  • Exceptional customer service, interpersonal and communication (listening, written and verbal), and liaison skills, with the ability to deliver dynamic, person-centred service within the community and in clients' homes.
  • Proven experience working in a fast-moving human services environment, managing a high workload, providing high quality advice and support for a range of client needs
  • A sound understanding of the needs and experiences of people who receive community care services and of the community care system in relation to the program
  • Competence and confidence in building productive networks and relationship development
  • An ability to take initiative, problem solve and make decisions
  • Experience working to operational key performance indicators (KPIs) and achieving business development targets, in a performance-driven environment
  • Experience managing client budgets, being responsive and accountable for time sensitive tasks
  • Drivers licence & own car
