We're looking for a dedicated Practice Nurse to join our Glenmore team in Rockhampton. You'll play a key role in structured care delivery and preventative health for patients with chronic conditions.
Pay Range: $35 – $50 per hour (dependent on chosen employment type & experience)

We're seeking an experienced and motivated Practice Nurse (Registered or Enrolled) to support our commitment to quality chronic condition management.
Role Overview
This dual-role practie nurse position includes dedicated care planning and chronic condition management as well as routine support in our treatment room. You will play a critical role in supporting General Practitioners to deliver structured care for patients living with chronic or complex health conditions under the updated Medicare Benefits Schedule (MBS) item numbers.
Key Responsibilities
· Develop and maintain comprehensive Chronic Condition Management Plans (formerly GPMPs) in accordance with the updated MBS framework.
· Conduct reviews, recalls, and patient follow-ups in line with CCMP protocols.
· Collaborate with GPs and allied health providers to ensure a coordinated and multidisciplinary approach.
· Educate and coach patients in self-management strategies, goal-setting and preventative care.
· Contribute to health assessments, wound care, immunisations and pre-employment checks as needed.
· Document all activity accurately using MedicalDirector and/or Pracsoft software.
You'll Bring
· AHPRA Registration as a Registered Nurse or Enrolled Nurse.
· Demonstrated experience in a general practice setting with chronic condition care planning.
· Strong familiarity with MBS item numbers.
· High-level communication skills and an empathetic, patient-focused approach.
· Capability to work both independently and as part of a team within a busy medical centre environment.
· Confidence using electronic health records and following RACGP-aligned best practices.
Highly Regarded
· Experience using MedicalDirector and Pracsoft.
· Experience supporting quality improvement activities or accreditation processes.
· Additional CDM training or chronic condition qualifications (desirable but not mandatory).
