About the Role

As a Family Support Worker, you will provide holistic person-centred support to families and/or individuals experiencing homelessness. You will work proactively with people to address complex barriers to achieve stability and independence. You'll collaborate closely with partner agencies to ensure a coordinated approach to support.  By delivering high-quality case management, you will focus on preventing homelessness, promoting well-being and building the foundations for sustainable tenancies and long-term resilience.

Key Responsibilities

Main tasks will include (but are not limited to):

  • Undertake comprehensive intake and assessment of families and individuals to identify needs, risks, and prioritise cases 
  • Provide holistic, person-centred support to families and individuals experiencing or at risk of homelessness
  • Develop  and implement individualised support and action plans focused on housing, health, wellbeing, education and financial stability
  • Deliver high-quality case management, maintaining regular contact and face-to-face visits, monitoring progress, and adapting  support as needs change
  • Work collaboratively with partner agencies to coordinate support and remove barriers
  • Advocate on behalf of clients to access housing, financial and specialist services
  • Maintain accurate and up-to-date case records, contributing to performance monitoring, and reporting, Empower clients to build resilience, confidence and independence, helping them to sustain tenancies and prevent repeat homelessness
  • Participate in team meetings, supervision, reflective practice, and ongoing training to ensure the delivery of high-quality support

About You

To be successful for this role, we're looking for someone with:

  • Proven experience working with families and/or individuals experiencing homelessness
  • A strong understanding of trauma-informed practice and person-centred  case management and multi-agency working
  • Excellent communication, and organisational and problem-solving skills
  • The ability to manage a varied caseload with professionalism, empathy and resilience
  • A genuine commitment to working as a team player, contributing positively to a supportive and collaborative team environment
  • Qualifications and/or experience in a human services framework for people presenting as homeless
  • Knowledge and understanding of the community and public housing sectors and services available to the local area  
  • Current Driver's Licence

Please note that a satisfactory police check is required for all staff and those with client related responsibilities are required to provide a NDIS and Working with Children Check (WWCC).
